What happens during a visit

We schedule around you
A member of our team calls or mails you to find a time that works. There's no pressure to decide right away.

A licensed clinician meets with you
A nurse practitioner or physician assistant reviews your health history, medications and current concerns with you.

A thorough look at your health
Medications, medical history, vitals when seen in person, memory and fall-risk screening, and anything on your mind, at your pace.

Keeping your doctor informed
A summary is provided to your health plan and your primary care physician, if you have one, so this visit adds to your care. It does not replace your regular care.
