Privacy Policy
Effective: October 2026EquiVera Health, Inc. ("EquiVera," "we," "us," or "our") partners with health plans, independent physician associations, and medical groups to support documentation integrity. This Website Privacy Policy explains how we collect, use, disclose, and protect information through our website at www.equiverahealth.com (the "Site"). It should be read together with our Website Terms of Use.
1. Scope of This Policy
This Policy applies only to information collected through the Site and through communications you start with us from the Site. The Site is intended primarily for business audiences, and it does not collect patient or member health information.
This Policy does not apply to health information we create or receive while providing clinical services. EquiVera provides those services on behalf of health plans, independent physician associations, and medical groups under written agreements, including business associate agreements. That information is protected health information ("PHI") governed by the Health Insurance Portability and Accountability Act of 1996 and its regulations ("HIPAA"), the California Confidentiality of Medical Information Act, and our agreements with our clients. It is described in the Notice of Privacy Practices issued by your health plan or medical group, not in this Policy.
2. Information for Health Plan Members
If an EquiVera clinician has contacted you or visited you, we did so on behalf of your health plan or medical group. Information about your health that we collect during a visit is used to complete and document your assessment and is shared with your health plan or medical group, and your primary care provider, as permitted by HIPAA and California law. We do not sell it or use it for advertising.
To learn how your health information is used, to request a copy of your records, or to exercise your other privacy rights, please refer to your health plan's or medical group's Notice of Privacy Practices or contact them directly. To ask us to stop contacting you, email us at info@equiverahealth.com. EquiVera will never ask you to pay for a visit and will never ask for your bank account or credit card information.
3. Please Do Not Submit Health Information
Our contact form, email addresses listed on the Site, and any chat feature are for business inquiries and opt-out requests only and are not secure channels for health information. Please do not include PHI, medical records, diagnoses, member or Medicare identification numbers, or other patient or member information. If you send us such information through the Site, we will delete it from our website systems and handle it as required by law.
4. Information We Collect
Information you give us. When you request information through our contact form or email us, we collect your name, organization, email address, and the content of your message.
Job applications. If you apply for a position through our online form, we collect the information you submit, such as your resume and contact information.
Chat. If the Site offers a chat feature and you use it, we collect the content of your chat and any contact information you provide.
Information collected automatically. When you visit the Site, our website platform and analytics providers automatically collect technical information, such as your browser type, device type, operating system, referring web address, pages viewed, time spent on pages, the date and time of your visit, and approximate location (city or region) derived from your Internet Protocol (IP) address. This information is collected through cookies and similar technologies described in Section 7.
Information from others. We may receive business contact information from clients, partners, professional networks, or public sources, such as when a colleague refers you to us.
5. How We Use Information
We use information collected through the Site to respond to your requests and communicate with you about our services; to evaluate prospective business relationships; to review job applications; to operate, secure, and improve the Site, including understanding how visitors use it and preventing fraud or misuse; and to comply with legal, regulatory, and contractual obligations and to establish or defend legal claims. We do not use information collected through the Site to make decisions about anyone's health care, coverage, or eligibility.
6. How We Disclose Information
We do not sell personal information collected through the Site, and we do not share it with third parties for their own advertising or marketing. We disclose it only as follows:
- Service providers. Companies that operate services for us, including website hosting, forms, and chat (Squarespace, Inc.) email and productivity (Google, LLC), website analytics (Google LLC), and IT support. These companies process information on our behalf to provide their services to us. Some of them, including Squarespace and Google, may also use certain information for their own purposes, such as security, fraud prevention, and improving their services, as described in their own privacy policies.
- Professional advisors. Our attorneys, accountants, auditors, and insurers.
- Legal and safety reasons. When required by law, subpoena, or court order, or when we believe in good faith that disclosure is needed to protect the rights, property, or safety of EquiVera, our clients, or others.
- Business transfers. In connection with a merger, acquisition, financing, or sale of assets, subject to protections at least as strong as this Policy.
- With your direction. When you ask us to share information or consent to it.
7. Cookies and Analytics
The Site uses cookies that are necessary for it to work, such as cookies that maintain security and remember your cookie preferences. With your consent where required, the Site also uses analytics cookies from Squarespace and Google Analytics to understand how visitors use the Site in aggregate. We configure Google Analytics without advertising features, Google Signals, or user-ID tracking, and Google Analytics 4 does not store full IP addresses. Google's use of this information is described at policies.google.com/privacy, and you can opt out of Google Analytics using Google's browser add-on at tools.google.com/dlpage/gaoptout.
We do not use advertising pixels, retargeting tags, or session-recording tools on the Site. You can accept or decline non-essential cookies through the cookie banner and cookie settings on the Site, and you can block or delete cookies in your browser.
Embedded content and social media. The Site may include embedded content and links from other services, such as videos, maps, and links to our LinkedIn page. Embedded content can set cookies and collect information about your visit when the page loads, under the provider's own privacy policy, even if you do not click on it. Following a social media link takes you to that service, where its own policy applies.
Do Not Track. Some browsers send "Do Not Track" signals. Because there is no common standard for responding to them, the Site does not currently respond to those signals. Third parties are not permitted to collect personally identifiable information about your online activities over time and across different websites through the Site, except as described in this Section.
8. Chat Feature
If the Site offers a chat feature, chat conversations are recorded and stored by EquiVera and our chat service provider and may be reviewed for quality, security, and recordkeeping. You will be communicating with an automated system or artificial intelligence. Chat is for business and general inquiries only. Please do not share health information in chat.
9. Phone and Email Communications
We do not use automated dialing systems or prerecorded messages to respond to Site inquiries, and we do not send marketing text messages without the prior express written consent required by the Telephone Consumer Protection Act and Federal Communications Commission rules. Our marketing emails identify EquiVera, include our postal address, and offer a working unsubscribe option, consistent with the CAN-SPAM Act. You may ask us to stop contacting you at any time.
10. Job Applicants
We use application information to evaluate your qualifications, screen applicants against federal exclusion lists maintained by the HHS Office of Inspector General and the System for Award Management (SAM), as Medicare program requirements call for, conduct reference checks where permitted by law, meet legal obligations, and manage any resulting engagement.
11. How We Protect Information
We use administrative, technical, and physical safeguards designed to protect information collected through the Site. No system is completely secure, and we cannot guarantee absolute security. If a security breach involves your personal information, we will notify you as required by California Civil Code section 1798.82 and other applicable law.
12. Your Choices
You may ask us to stop contacting you, by emailing info@equiverahealth.com. We will respond within a reasonable time, subject to information we must keep by law. We do not disclose personal information to third parties for their direct marketing purposes.
13. Children
The Site is not directed to children, and we do not knowingly collect personal information from children under 13. If you believe a child has given us personal information, please contact us and we will delete it.
14. Links to Other Websites
The Site may link to websites operated by others, including health plans, partners, and government agencies. We are not responsible for their privacy practices, and we encourage you to read their policies.
15. Accessibility
If you need this Policy in an alternative format because of a disability, please contact us using the information below.
16. Changes to This Policy
We may update this Policy from time to time. We will change the "Effective" date above and, for material changes, provide notice on the Site. Your continued use of the Site after an update takes effect means you acknowledge the updated Policy.

Effective: October 2026These Terms of Use govern your use of the website at www.equiverahealth.com (the "Site"), operated by EquiVera Health, Inc. ("EquiVera," "we," or "us"). By using the Site, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site.
1. About the Site
The Site provides general information about EquiVera and the services we offer to health plans, independent physician associations, and medical groups. It is intended primarily for business audiences; Section 2 contains information for health plan members. Using the Site or contacting us through it does not create a client relationship. Our services are provided only under a signed written agreement, and if that agreement conflicts with these Terms, the signed agreement controls.
2. Information for Health Plan Members
EquiVera clinicians provide in-home and telehealth visits on behalf of health plans and medical groups. If you are a member who has been contacted or visited by EquiVera, please know that visits are voluntary and you may decline or reschedule at any time. EquiVera will never ask you to pay for a visit and will never ask for your bank account or credit card information. To confirm that a call or visit is from EquiVera, please contact your health plan. To ask us to stop contacting you, email us at info@equiverahealth.com. Questions about your care or your health information should be directed to your health plan or independent physician association.
3. Not Medical, Legal, or Coding Advice
Content on the Site is for general information only. It is not medical advice, and using the Site does not create a provider-patient relationship. It is also not legal, coding, billing, or compliance advice. Descriptions of our services reflect our processes and standards, not a promise of any particular result. If you are experiencing a medical emergency, call 911.
4. Do Not Submit Health Information
Our contact form is for business inquiries only. Please do not submit protected health information or any patient or member information through the Site.
5. Chat Feature
If the Site offers a chat feature, these Terms apply to its use. By using chat, you consent to EquiVera and its chat service provider recording, storing, and reviewing your chat communications for quality, security, and recordkeeping purposes. Chat responses are provided by an automated system or artificial intelligence. Chat is for general and business inquiries only. Do not share protected health information or any patient or member information in chat, and do not use chat for medical questions or emergencies.
6. Cookies and Analytics
The Site uses cookies and analytics tools as described in our Privacy Policy. You can manage non-essential cookies through the Site's cookie settings or your browser.
7. Job Opportunities
Job postings on the Site are for information only. They are not offers of employment or engagement and do not create a contract. Any employment or independent contractor relationship is established only by a separate written agreement. EquiVera never charges applicants a fee and never asks applicants for payment, bank information, or equipment purchases during recruiting. If you receive such a request in EquiVera's name, do not respond, and please contact us. Information you submit when applying is handled as described in our Privacy Policy.
8. Our Content
The Site and its content, including text, graphics, logos, and the EquiVera name and marks, belong to EquiVera or its licensors and are protected by intellectual property laws. You may view and print pages from the Site for your own business reference. Any other use, including copying, modifying, or distributing Site content, requires our written permission.
9. Client Names, Testimonials, and Results
Names and logos of other organizations that appear on the Site belong to their respective owners and are displayed with permission. Their appearance does not mean those organizations endorse EquiVera except as expressly stated. Testimonials reflect the experience of the organizations that provided them. Results and statistics describe past performance under specific programs and conditions, may not be typical, and do not guarantee future results.
10. Acceptable Use
You agree not to use the Site for any unlawful purpose; attempt to gain unauthorized access to the Site or its systems; interfere with the Site's operation; use automated tools to scrape or collect content from the Site; use Site content to train, fine-tune, or develop artificial intelligence or machine learning models without our written consent; or use our name or marks in a way that suggests our endorsement without our written consent.
11. Links to Other Websites
The Site may link to websites operated by others. We do not control those websites and are not responsible for their content or practices.
12. Disclaimer
THE SITE IS PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED OR ERROR-FREE, OR THAT ITS CONTENT IS COMPLETE OR CURRENT.
13. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, EQUIVERA WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR SPECIAL DAMAGES ARISING FROM YOUR USE OF THE SITE, AND OUR TOTAL LIABILITY FOR ANY CLAIM RELATING TO THE SITE WILL NOT EXCEED ONE HUNDRED DOLLARS ($100). This section applies only to use of the Site and does not affect any signed services agreement.
14. Governing Law
These Terms are governed by California law. Any dispute relating to the Site will be brought in the state or federal courts located in Alameda County, California. Any claim relating to the Site must be brought within one (1) year after it arises, to the extent permitted by law.
15. Accessibility
We aim to make the Site accessible to everyone, including people with disabilities, and we work toward conformance with the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA. If you have difficulty accessing any part of the Site, please contact us using the information below and we will work with you to provide the information in an accessible format.
16. Nondiscrimination
EquiVera complies with applicable federal and California civil rights laws and does not discriminate, exclude people, or treat them differently on the basis of race, color, national origin, ancestry, religion, age, disability, sex, sexual orientation, gender identity, or any other characteristic protected by law, in its services, programs, or employment.
17. Changes and Severability
We may update these Terms or the Site at any time. Changes take effect when posted, and your continued use of the Site means you accept them. If any provision of these Terms is held unenforceable, that provision will be enforced to the maximum extent permissible, and the remaining provisions will remain in full effect.
