Common Questions

What health plans and IPAs do you work with?

We partner with health plans and independent physician associations serving Medicare Advantage patients in California. Each engagement is scoped to the partner's population, network and existing quality programs, so our work fits alongside what is already in place.

Assessments are conducted by nurse practitioners and physician assistants in the patient's home, in a clinic, at health fair events, in skilled nursing facilities, by video or as a hybrid visit, depending on what works best for the patient. Each in-person visit includes a health history, physical exam, medication review and screening for social needs. Diagnoses are documented with supporting clinical findings, coded to current HCC and ICD-10 standards, and provided to the health plan or IPA.

Our clinicians document each diagnosis with the clinical findings that support it, coded to current HCC and ICD-10 guidelines. Each assessment looks both ways: confirming conditions that are clinically supported, and flagging diagnoses that are outdated or unsupported for removal from the patient's medical record documentation. We also audit our own notes.

Timelines vary with population size, credentialing and data integration needs. We share an estimated timeline during scoping and complete setup steps in parallel where possible.
